Don't know how the failure happened, but I'd guess it was due to prying.
I stole these pics from another forum, like a common thief.
If the original poster of these images objects to their dissemination, please notify me and I'll remove them.

Wow that's a real shame!
One of the worlds best knives in my opinion and it's broke.
I wonder what happened? No explanation I am assuming went with the pic?

JB Weld?
These things happen. I saw a similar breakage with a D2 Military over at bladeforums a few years ago. IIRC the poster was prying open the flaps of a cardboard box that were sealed with adhesive. Seemed like a basic flaw in the steel, similar to what we see with broken plier heads. Not really indicative of overall strength or anything else just the nature of mass produced steels.
If you look at the second picture it looks like the first inch of the blade was stuck into something and then pried. There fulcrum point was deep enough that the tip didn't break so the next weakest area at the hole went.
Are they the same knife? Both pictures are different composure and the first looks distinctly like a Carbon Fiber model while the second looks like G10?
Should have gone with H1 instead.

Any blade can end up like that if you miss-use it. Take this as a warning people.
