This one was only a cock-up from the doers perspective, but nevertheless:
We had this collegue who claimed he never messed up. And to be fair he was annoyingly right. One slow day, of all things, he decided that he would fix a leak wheelbarrow wheel by changing the hose inside. So he goes off to some shop to find a replacement.
He comes back, changes the hose, and it is still leaking. Must have been a bad replacement hose. Swearing a bit, but off to the shop again to get a proper one.
He comes back, changes the hose, but it is still leaking. Swearing more creatively and louder, checking all the tools for possible sharp bits, but still no clue what is wrong. Off to the shop yet again - must be some old trash bad brand smurfy stuff for sure!
Comes back, repeats everything with great care, and same result - the wheel is still leaking. Walks away, refuses to talk about it, and claims from the next day forward it never ever happened. Never claimed to never make cock-ups again.
Like I said it was only a cock-up from his perspective though: It was a slow day so we had plenty of time too. While one would discuss this devilish wheel with him to distract, someone else would switch the hoses. That way he kept putting back the original bad one.
I still don't know how we managed to do that while keeping straight faces
