There's already the black sticker with the "made in China" and the LTG part number on the inside, but when the put the wrong sticker in the sheath ...  ::)

Um yes. One can see why the packers could get confused. :think:
Vic solved this problem long ago with different colours of trim (see Swisstool vs Spirit nylon sheaths). Maybe a tip for LM? It could be as simple as a reversed colour scheme on the sewed-on logo...

The sheaths look very alike, and the only difference is about 1 or 1.5 cm in length, but still, this shouldn't be a problem for the packers (if they know what they're doing, at least).  Leather sheaths (or stiffer Nylon, like the Nylon Vic sheaths) are easier to tell the difference in size.  With the "new" nylon Leatherman sheaths it's harder since you can change the shape of them a bit.  I mean, when I place my medium sized sheath on my workdesk, and I apply some pressure on it the sheath flattens and seem to become a bit longer.  That might be enough to mix them up.

With the previous Nylon sheaths from Leatherman , you could tell the difference since the large sized sheath had a cut-out on the top so you could put a bigger (longer) flashlight in the side pocket, and the medium sheath did not have this, else, they would've had the same problem I guess.

Has anyone bought one of these recently? I need a new medium sheath. I'm using a "medium" for my wave right now and it's super loose in there.

I've bought the 4 pocket ones some 2 years ago, like you I guess. And had the exact same problem.
Because I've bought a Medium and a Large I can say that it was NOT a labelling issue.
They were both differently made in respect to sizing and placing of buttons etc. Still, the Medium one was nearly as long.

The Medium sheath was quite a bit too long, while the Large sheath could have been a bit longer (had to stretch it to fit my Surge plus bits).

I've solved the problem with the Medium sheath the "McStichy" way  :D

Wave and Rebar fit perfectly in now.

Can't explain what went wrong, all I know is that NKlamerus his Medium sheath could hold his Surge, while mine could not...

Maybe they scr*wed up a batch (or all of them in the beginning and mine came from a new badge, who knows?

Anyway, I'm still happy with mine.

It is best to put the bit card in it along with the Charge/Wave, or else it's a bit loose (but nothing like the ones you guys got).  But that was the idea of this sheath when I bought it (to carry a Charge/Wave with bit card and bit extender). 

Only thing I find a bit weird is the fact that, from time to time, there are these thin red pieces of sewing thread on the side of the sheath, which can be pulled out without having to cut them, still, no idea where they come from.
