I think that the combo tool can be swapped for a small blade
But i dont know about the ather quistions

Depends on which combo tool version. Is it 84mm or 91mm?

A 91mm will not fit on a 84mm frame. However a 84mm combo tool will fit on a 91mm. Also it should stop if the combo tool is flat on the side.

Would it be better to find a 91mm combo tool for a 91mm knife I am thinking of doing?

IMO only yes I would. Might I suggest a marlboro sak. They have the 91mm combo tool.

I picked up a couple 91 mm Compacts, then have the combo tool as well.
