A friend of mine ran into this and he says the peening seems to be the factory one... did anyone see something like this before?

No idea but this is interesting!

Do only the straight edge and cap lifter lock?

It looks like it has dual liner locks, so I'd imagine the OH and THO blades and the caplifter should all lock.  Very weird piece.
