Blade exchanger issue

So today the blade exchanger developed a problem. When deploying the tool, the blade holder comes out, but the cover stays behind. Here's a link to a picture:

Anyone else have this problem? I had to tighten the pivot slightly the other day because the knife blade had too much play for my liking, but I loosened it back and the exchanger still had the issue.

It happened to me a few times when my Surge was still new. I lubbed the pivot but learnt that I had to open the exchanger with my nails where the little dent is to get a proper deployment.
