The usual way was to get them in these hard plastic shells that have a built in belt clip. You could remove the belt clip and slide them into a specially made leather sheath as well.
The Left is the older "Standard" adapter designed to works with older Leatherman tools such as the PST, PST II, Supertool, Minitool, Sideclip, Flair, Pulse, etc. The right is the newer "Universal" adapter designed to work with the newer Wave, Core, Charge, Blast, Fuse, and Kick.
There was also a more integrated solution for those wanting to carry everything in one, and that was this unified sheath. I have included a PST for photo purposes, but the adapter is made to fit the newer Leatherman models.
The Leatherman Tool Adapter also came in a desktop display carrier which also came with a special leather sheath. This model is however a bit bulky for most folks to wear on their belt.
