"Needed" might be to much, but i remember situations (outdoors, not attacking medieval castles) where they would have been handy.I just don´t do such stuff anymore.

Too dangerous to climb. Defenders can simply cut the rope and attackers have both hands occupied and are vulnerable. Slap up a ladder and the attacker can have one hand free or even both hands free in some situations . Plus, it's much harder for defenders to dislodge the ladder.
Grappling hooks were used a lot in the age of sail in ship to ship engagements, and for more practical day-to-day ship operations. Solid ones, though... not folding ones. Also, I feel a strong need to append the character list to Batman, Rorshach, and Luke Skywalker. Just sayin'.
Hmmm, we were trained with them for urban combat.Group gives covering fire to a buildings windows, one man (usually one of the lighter ones in the group) sneaks to the building. Throwing a handgrenade into the choosen window in a upper storage, then the grappling hook. Climbing up just under the window, auto fire into the room, going in. Securing the hook to ensure it´s fit for the more heavy comrades while securing the door. Second man climbs up, then the third ..... then searching and cleaning the building.Off course that throwing the grenade and hook plus climbing and securing the room has to be done fast, too. No time for putting on any kind of climbing rig, just go up the doubleline ropes with knots every 30-40cm.Ladders of different types were also trained, you use whats available and suits the situation best.I would have loved to use them in situations like crossing a fast flowing river. As a securing thing. It´s one thing to have a rope on you that is hold by friends to pull you back if needed, but its nicer to have a second line already on the other side, to help you keep a little bit safe(r) stand, or pull yourself over while swimming. It could help the first one crossing, then he can secure the rope on the other side to help the others.
At least for me, you couldn't pay me enough to take that risk.

Given a low enough window, and a high enough payday, I'd do it.Second story window.Would you do it for a thousand? Lynn: Are you crazy?Would you do it for two hundred million?Lynn: Up I go!!!!Unlike many other people, I CAN be bought. Heck, for two hundred million, I'd sing the praises of the Engage MT!
