As for what did it, you could give us a clue. Most of the things we discuss on here could do that :D

It was done with a cheap ass box cutter. Just put a new blade in it too.  :ahhh

If it was a new blade in a box cutter it probably doesn't hurt as bad as it looks. Although I'm sure it's a bit sore. Getting cut by a dull knife is a lot more painful than getting cut by a sharp one.

At least a new blade is likely to be relatively clean too.  That looks pretty nasty though... maybe they should put a warning label on those things, like "WARNING- Using this product to slice into your fingers may cause injury."

Is there any tendon damage in that knuckle?  That can keep the healing process going for months.  I cut into the joint of my little finger once and it took about 6 months before it was finally better.  It looked better after a week or two, but every time it brushed up against anything it sent jolts of pain all the way up my arm!

Did the same thing cutting drywall one day almost took the tip of my finger off. Cut thru two fingers and a finger nail. No stitches (too stubborn to go) all healed up nicely though

Damn. Cuts across the knuckles, they hurt every time you close your hand.  :(

Ouch! To me the cut is more likely done by the blade of a multitool, its not clean and straight enough to be cause by a box cutter!
Is this a contest those who guess it right will get a free trip to the ER :D

Damn!

Hope you heal well and there was nothing but skin that was cut.

The worst tool injury I ever had was not with a blade at all. I was cutting trim with a miter saw and the blade guard sucked into the blade locking it up in a millisecond. This caused the work piece to splinter and fly out at what must have been 100 mph. It hit my thumb from the palm side and treated the joint like a hammer treats a glass marble. Bone dust and fragments were visible through the open flap of skin. My thumb looked like you took it off and put it back on at a jaunty angle and backwards. Light was also visible through the opening the wood made on the impact side.

The nurse was nonchalant at the ER until she asked to see the wound. Then she went white as a ghost and screamed, "I need a trauma room stat!"

The drugs were good.

It works at about 60% flexibility as compared to pre-injury, and there is a nasty scar. It still has the jaunty angle too. They did the best they could I suppose. 

Man, hope you heal up fast. Injuries to the hands suck more than anything.
