Leatherman Surge gets you pretty much there. Or Zap Wizards mods for the LM Free  although I'm not sure how many you can fit in there. The ST300 is also pretty mod friendly.
The Scalpel holder is going to be the hard one on any tool other than the Free since one is available already for it.

Welcome to  :MTO:

:iagree: with Steve :tu: Seems like someone added a scalpel holder to a Wave or Surge before but I can't remember who  or when

Leatherman Surge gets you pretty much there.

I did not know that the surge had a tshank holder..
Can I use thinner blades in that one that is like 1mm thin?

Gotta agree with SteveC and Poncho65 on this, Surge is good cause it has allready have t-shank adapter and good pliers.
Do you want full 1/4” adapter or will flatbit adapter do fine. It could be hard to find good 1/4” adapter to it, 3d printed adapters wont hold much force on them.
ST300 EOD has t-shank adapter, i could say that all version of ST300 easy to mod.

I dunno how easy is then to mod Gerber cause i only own CenterDrive, but you could give look at MP800 ( has u-shank adapter), MP1 ( has 1/4" adapter) or MP600 where you could add t-shank or u-shank and hexbit driver 1/4" adapter

The thinner blades work :tu: there is just a bit of play but that can be fixed by wrapping the end with either cellophane or some types of tape like painters tape or electrical tape  I never wrap them myself though and have used them as is :salute:
